I'm working on a new patch that isn't quite so….messy. This new one should just enable the Extra Options in the Options menu, as well as (hopefully) fixing widescreen HUD stretching and black bars on certain TVs if you're playing on real hardware. Also added an .xDelta patch since .IPS is fickle with bigger game sizes.

This patch removes the noise effect/film grain present throughout the game. Reddit user u/enscripture made the original patch years ago but it doesn't seem to actually do...
