This is an MS-DOS boot disk containing drivers to utilize a FlashPath adapter.
1.) Write the "FLASHPATH.IMA" file using WinImage onto a 720kb floppy disk or above.
2.) Insert the disk into the machine that you are intending to boot the disk from.
3.) Power on, then select the boot option from floppy.
4.) When asked, swap the boot disk with the FlashPath adapter, then do a "DIR /W" to ensure that the files are there.
